DIEP Flap Reconstruction 

The DIEP (deep inferior epigastric perforator) flap reconstruction technique uses tissue from the lower abdomen to recreate the breast, offering a natural look and feel while preserving muscle function in the core. Plastic Surgeons of Alaska is currently the only practice in the state to offer this advanced breast reconstruction option.

Your surgeon performs this procedure while you are under general anesthesia. They will remove a section of skin, fat, and blood vessels from the lower abdomen and transfer it to the chest. This tissue is connected to a new blood supply and shaped into the breast mound. Your surgeon will then suture the incisions. 

In most cases, patients need a hospital stay of 3 to 5 days after the surgery. Your surgeon will provide you with detailed instructions regarding wound care and activity restrictions. You will need to wait several weeks for your abdomen and breast(s) to heal fully and for residual swelling to dissipate. 

Advantages and Disadvantages of DIEP Flap Reconstruction 

Advantages 

One of the most significant benefits of DIEP flap reconstruction is its natural appearance. By using the patient’s own tissue, the reconstructed breast closely mimics the look and feel of a natural breast, offering a more aesthetically pleasing result than an implant only reconstruction

DIEP flap surgery also preserves the abdominal muscles, which means patients retain their core strength and function. This contrasts with other techniques, such as TRAM flap reconstruction and latissimus flap reconstruction, where muscle removal can impact abdominal strength. 

In addition, the procedure not only reconstructs the breast but can also enhance the contour of the abdomen. Removing skin and fat from the lower abdomen can lead to a flatter, more toned appearance. 

DIEP flap reconstruction is a popular breast reconstruction option for its long-term durability. Unlike implants, which can rupture, develop capsular contracture, or require replacement, the DIEP flap uses the patient’s own tissue, eliminating the risk of these complications.

Disadvantages 

DIEP flap reconstruction is a complex procedure that typically takes longer than implant-based reconstruction. The intricate nature of the surgery, involving microsurgical techniques to connect blood vessels, can extend the duration of the operation. 

Consequently, DIEP flap recovery is more extensive as both the abdominal and breast areas need time to heal. Patients may experience discomfort and require longer healing time than with other reconstruction methods. 

As with any surgical procedure, DIEP flap reconstruction involves scarring. Patients will have scars on both the abdomen and the reconstructed breast. While the abdominal scar is often placed along the bikini line to minimize visibility, it can still be noticeable. 

Lastly, not all patients are candidates for DIEP flap reconstruction. The procedure requires sufficient abdominal tissue to harvest, making it unsuitable for individuals with previous abdominal surgeries or limited abdominal fat. Additionally, certain health conditions may preclude this option. Your board-certified plastic surgeon will help you determine your candidacy and the most appropriate reconstruction technique.
